Feedback forms, often questionnaires or surveys, are useful for the instructor while developing or modifying a course. They are short surveys with items related to how the course is running, perceived changes, impact upon the student, etc. They do not count toward the students’ grades and may be administered several times during the semester. Detailed description on questionnaires of this type can be found on the Evaluation Cookbook site, Institute for Computer Based Learning, School of Mathematical and Computer Sciences at Heriot-Watt University, Scotland.
Feedback Form: a final course assessment with short-answer questions designed to help the instructor improve the course itself, including content, delivery method, and external projects, from Environmental Science 110, Understanding Earth, University of Missouri, Kansas City.
